Room Reservations:
To receive the special conference rate listed above, visit www.stfm.org/annualconf/hotel.htm or call Hyatt's reservations department at 888-421-1442 and tell them that you are with the STFM Annual Spring Conference. Reservations must be made by April 6, 2006; after that date, rooms will be assigned on a space-available basis.
If you would like to share a room, call STFM at 800-274-2237, ext. 5415. You will receive information for other persons interested in having a roommate, from which you can then make your own room-sharing arrangements.
Airfare:
Passport Travel, STFM's official travel agency, is available to help you with your airline arrangements. For travel assistance and other special services, call 800-417-2902 and ask for the STFM Travel Desk. Office hours are 9 am-5 pm (CST) Monday through Friday.
Rental Car:
Budget Rent A Car System, Inc. is the official rental car agency for this year's conference. For reservations, call Budget at 800-772-3773, or you can make your reservations on-line at www.budget.com. Be sure to use the following convention discount code for our conference when making your reservation: U063655. Rates for the conference begin at $38 daily/$136 week/$24 weekend day. All rates include unlimited mileage and are valid for up to 1 week before/after the conference.

Child Care:
Children's programs and daycare services are available by calling the Hyatt concierge desk. Services should be reserved 24 hours in advance. For more information, or to make arrangements, call 415-788-1234.
Shuttle Service/Ground Transportation:
Shuttle service to the Hyatt from the San Francisco Airport is available on the upper level of the terminal, located outside of the baggage claim areas. Shuttles depart from the center island outside of the terminal. The fare is $15 per person, one-way. Taxi service from the San Francisco airport to the hotel is approximately $50 one-way. Please check rates with the taxi driver before leaving the airport. The Hyatt is approximately 30-40 minutes from the airport.
CME Hours:
This program has been reviewed and is acceptable for up to 29 Prescribed credit hours by the American Academy of Family Physicians. This includes 29 Prescribed and 7.75 Elective credits. Because some sessions run concurrently, no more than a total of 29 credits may be reported.
Continuing education credits have been requested for psychologists, social workers, professional counselors, MFCCs, and health care workers. Please view this pdf for more information.
This program is approved by the American Osteopathic Association for Category 2 credits for DO participants.
For other credit, STFM will assist individuals by providing information needed to the extent possible.
Cancellation Policy
50% of the total registration fee will be refunded if a cancellation is necessary and written notification is received in the STFM office by March 24, 2006. After that date, no refunds will be issued.
