1. Save the file to your computer. To do this click on "DOWNLOAD FORM". This will automatically launch the form into your word program and then you can save it. Or right-click on "DOWNLOAD FORM" and select "save target as.." or "save link as.." and then save the file.

2. Open the document in Word or some other word processing software on you computer.

3. Type in the appropriate information. Use your tab key to move to various fields thoughout the document. Use an "x" or hit the spacebar to check off the boxes.

4. Save the document, so you'll have a copy for your records.

5. Once completed, you may email this form as an attachment to assndfm@stfm.org. Please put "2001 Spring Conference Registration" in the subject line.
