Conference Registration:
Register on or before February 9, 2004, to receive the lowest registration fee. Fees are an additional $25 per registration if you register after February 9th or on site. Your conference registration fee covers all educational sessions except preconference workshops.
Room Reservations:
Reservations must be made by January 23, 2004; after that date, rooms will be assigned on a space-available basis. Rates do not include state and local sales tax. (Please note: Amelia Island Resort will add a $7 service fee per night to cover all gratuities and extra resort services including in-room coffee service, Island shuttle service, daily housekeeping, and use of the health and fitness center.)
Reservations must be guaranteed for late arrival with a credit card. Cancellations must be made at least 24 hours prior to arrival date. Check-in time is 4:00 pm and check-out time is 11:00 am. Please note, if conference attendance exceeds expectations, rooms in the conference block may be sold out prior to the cut-off date of January 23, 2004.
Continuing Education Credits
This program has been reviewed and is acceptable for up to 18.25 credit hours by the American Academy of Family Physicians. This includes 18.25 Prescribed and 1 Elective credit hours. Because some sessions run concurrently, no more than a total of 18.25 credit hours may be reported.

This program is available for up to 23 continuing education (CE) credits through the co-sponsorship of PsychoEducational Resources (PER) and the Society of Teachers of Family Medicine. PsychoEducational Resources, Inc. (PER) is approved by the American Psychological Association to offer continuing education credits for psychologists. PsychoEducational Resources, Inc. (PER) maintains responsibility for this program.
PER is an NBCC approved provider (5536) and adheres to NBCC sponsorship guidelines. This program is acceptable for NBCC continuing education credits. PER is approved as a provider for social work continuing education (Provider #1008) by the Association of Social Work Boards, (800-225-6880) through the Approved Continuing Education program. PER is a NAADAC approved education provider, #374 (Expiration 10-31-05).
The individual sessions in this conference cover the following counselor skill groups: clinical assessment, ongoing treatment planning, counseling services, documentation, case management, and legal, ethical, and professional growth issues.

PER maintains responsibility for this program and adheres to all sponsorship guidelines for awarding CE credits.
Air Travel
Shorts Travel, STFM’s official travel agency, has negotiated discounted fares for this conference. To take advantage of these discounted fares and other special services, call 800-417-2902 and ask for the "STFM Travel Desk." Office hours are 9 am–5 pm (CST) Monday through Friday.
When making your travel arrangements, please keep in mind that Amelia Island is approximately 29 miles from Jacksonville International Airport.
Rental Car Discount:
Alamo Rent-A-Car, Inc is the official car rental provider for the STFM Conference on Families and Health at Amelia Island, Fla. Alamo is offering special discounted rates to all attendees, including unlimited mileage. Rates and rental cars are based on availability from the Jacksonville (FL) Airport. Rates begin at $40/day or $179/ weekly.

Child Care:

Spouse Activities:
Various recreational opportunities are available for spouses. An orientation for families will be held on Thursday morning.
Shuttle Service/Ground Transportation:
Roundtrip service from Jacksonville (Fla) International Airport is available with Amelia Island Resort Shuttle Service (green van). Roundtrip fare is $36/one way for adults; $15/one way for children (12 years and under). To make reservations, call 904-261-6161, ext 5244. Reservations are required at least 24 hours in advance. Drivers will meet you at the “Ground Transportation” area located outside of the baggage claim area. Taxi service from the airport to the resort will range from $40-$50/one way.
