Fees are an additional $25 per registration if you register after February 9th or on site. Your conference registration fee covers all educational sessions except preconference workshops. Room Reservations: Reservations must be made by January 23, 2004; after that date, rooms will be assigned on a space-available basis. Rates do not include state and local sales tax. (Please note: Amelia Island Resort will add a $7 service fee per night to cover all gratuities and extra resort services including in-room coffee service, Island shuttle service, daily housekeeping, and use of the health and fitness center.) Reservations must be guaranteed for late arrival with a credit card. Cancellations must be made at least 24 hours prior to arrival date. Check-in time is 4:00 pm and check-out time is 11:00 am. Please note, if conference attendance exceeds expectations, rooms in the conference block may be sold out prior to the cut-off date of January 23, 2004. Continuing Education Credits This program has been reviewed and is acceptable for up to 18.25 credit hours by the American Academy of Family Physicians. This includes 18.25 Prescribed and 1 Elective credit hours. Because some sessions run concurrently, no more than a total of 18.25 credit hours may be reported. This program is available for up to 23 continuing education (CE) credits through the co-sponsorship of PsychoEducational Resources (PER) and the Society of Teachers of Family Medicine. An orientation for families will be held on Thursday morning. Drivers will meet you at the “Ground Transportation” area located outside of the baggage claim area. Taxi service from the airport to the resort will range from $40-$50/one way.
