Conference Rates:

Resortview Room $128 single/double
Oceanview Room $179 single/double

Conference Registration:

Register on or before February 1, 2005, to receive the lowest registration fee. Fees are an additional $25 per registration if you register after February 1st or on site. Your conference registration fee covers all educational sessions except preconference workshops.

Room Reservations:

Reservations must be made by January 14, 2005; after that date, rooms will be assigned on a space-available basis. Rates do not include state and local sales tax. (Please note: Amelia Island Resort will add a $7 service fee per night to cover all gratuities and extra resort services including in-room coffee service, Island shuttle service, daily housekeeping, and use of the health and fitness center.)

Reservations must be guaranteed for late arrival with a credit card. Cancellations must be made at least 24 hours prior to arrival date. Check-in time is 4:00 pm and check-out time is 11:00 am.

Continuing Education Credits

Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit hours is pending.

Air Travel

Short's Travel, STFM's official travel agency, has negotiated discounted fares for this conference. To take advantage of these discounted fares and other special services, call 800-417- 2902 and ask for the "STFM Travel Desk." Office hours are 9am-5pm (CST) Monday through Friday. NOTE: We encourage you to make your flight arrangements early since the conference is held at the time of year when many schools are beginning their winter-spring breaks!

When making your travel arrangements, please keep in mind that Amelia Island is approximately 29 miles from Jacksonville International Airport.

Rental Car Discount:

Budget Rent A Car System, Inc, has been selected as the official rental car agency for this year's conference. For reservations, please call Budget at 800-772-3773, or you can make your reservations on-line at www.budget.com. Please be sure to use the following convention discount code for our conference when making your reservation: U063655. Rates for the conference begin at $37 daily/$114 weekly, with special weekend rates available beginning at $30 per day! Rates include unlimited mileage and are valid for up to 1 week before/after the conference.

Child Care:

Children’s programs and day care services are available
by calling the resort concierge desk. Services should
be reserved 24 hours in advance. For more information,

Shuttle Service/Ground Transportation:

Roundtrip service from Jacksonville (Fla) International Airport is available with Amelia Island Resort Shuttle Service (green van). The fare is $36/one way for adults and children over 12. Children 12 and under ride for free. To make reservations, call 904-261-6161, ext. 5244. Reservations are required at least 24 hours in advance. Drivers will meet you at the "Ground Transportation" area located outside of the baggage claim area. Taxi service from the airport to the resort will range from $40-$50/one way.
