Room Reservations:
Please make your reservations directly with the Hyatt Regency San Francisco. Be sure to request the special rate for the Conference on Patient Education when making your reservations. The deadline for reservations within the block of rooms for this conference is October 11. However, if conference attendance exceeds expectations, rooms in the conference block may be sold out prior to this date. Please make your reservations as soon as possible to ensure your preferred accommodations. After October 11, rooms will be on a space- and rate-available basis.

Rental Car Discount:
Budget Rent A Car System, Inc, has been selected as the official rental car agency for this year's conference. For reservations, please call Budget at 800-772-3773, or you can make your reservations on-line at www.budget.com. Please be sure to use the following convention discount code for our conference when making your reservation: U063655. Rates for the conference begin at $35.99 daily/$130.99 weekly, with special "weekend rates" available beginning at $21.99 per day! Rates include unlimited mileage and are valid for up to 1 week before/after the conference.

Shuttle Service/Ground Transportation:
Shuttle service to the Hyatt from the San Francisco Airport is available on the upper level of the terminal, located outside of the baggage claim areas. Shuttles depart from the "center island" outside of the terminal. The fare is $14 per person, one-way. Taxi service from the San Francisco airport to the hotel is approximately $50 one-way. Please check rates with the taxi driver before leaving the airport. The Hyatt is approximately 30-40 minutes from the airport.
CME/CEU Credit:
Physicians -
This program has been reviewed and is acceptable for up
to 20.0 credits by the American Academy of Family Physicians.
This includes 20.0 Prescribed and 3.0 Elective credits. Because
some sessions run concurrently, no more than a total of
20.0 credits may be reported. This activity has been planned
and implemented in accordance with the Essentials and
Standards of the Accreditation Council for Continuing
Medical Education through the joint sponsorship of the AAFP
and the STFM. The AAFP is accredited by the Accreditation
Council for Continuing Medical Education to sponsor
continuing education for physicians. The AAFP designates this
educational activity for a maximum of 20.0 hours in Category
1 credit toward the American Medical Association Physician's
Recognition Award. Each physician should claim only those
hours of credit that he/she actually spent in the educational
activity.
Nurses, Dietitians, and Health Educators -
Application for other continuing education credits will be
made to appropriate organizations. For more information,

will be confirmed in October.
Cancellation Policy:
Written cancellations received before October 11 will qualify for a full registration refund. Written cancellations received between October 11 and October 25 will be eligible for a 50% refund. No refunds will be made for cancellations received after October 25.
