2008 Medical Economics Award
for Innovation in Practice Improvement

This annual award will be presented to a health professional or not-for-profit organization involved in developing programs and strategies for innovation in practice improvement by transforming office processes, promoting patient participation, and contributing to an office practice's overall success. The award seeks to acknowledge creative, cutting-edge strategies developed to effectively redesign an office-based practice and promote patient participation and practice growth.. In addition to innovation and creativity, submissions will be judged on the demonstrated capacity to streamline office and patient processes, and to positively impact patient behavior. Individuals or groups may apply. Submissions from interdisciplinary teams/practices, students and residents are encouraged. Nominations will also be accepted.

The award winner will receive a commemorative plaque, plus a cash award of $500, and a $300 stipend for travel expenses to the conference. The winner will also receive a complimentary conference registration. The award will be presented during the Conference on Practice Improvement, December 4-7, 2008 in Savannah, GA. The winner is expected to be present for the award presentation and to prepare a description of their program for the award citation printed in the conference program.

4. Individuals, practice groups or organizations who have been awarded a Patient Care Award at the Conference on Patient Education or Conference on Practice Improvement within the past three years are not eligible to submit for this award.

2008 H. Winter Griffith Award for Excellence in Practice Improvement Involving Patient Education

This award recognizes excellence in practice improvement that involves patient education produced by an individual or organization. The practice improvement activity should document a measured change in patient knowledge, attitudes, skills or environment that has either improved or has a high probability of improving measures of patient health behavior or measures of health, such as blood pressure, blood sugar, obesity or frequency of preventable emergency room visits. The patient education activities can include print, internet based education or monitoring, and/or electronic media, including DVDs, CDs, etc.

Awards may be issued for both categories:

1) Individual (residency program, group practice, small rural hospital, etc.)

2) Organization (large hospital, pharmaceutical manufacturing company, association, university, etc.)

The winning submission(s) will receive a commemorative plaque, a cash award of $500, a $300 stipend for travel to the conference, and a complimentary conference registration. The award(s) will be presented during the Conference on Practice Improvement, December 4-7, 2008 in Savannah, GA. The winners are expected to be present for the award presentation and prepare a descriptive/biographical information for the award citation for the final program.

ELIGIBILITY
1. Entries are limited to documented practice improvement activities that involve patient education programs or patient education materials which have been produced in their current format within the last three years.
2. Only one entry may be submitted annually by an individual or organization.
3. The entries must permit individualization for specific patients either through electronic methods or through space for comments of special instructions (may be wide margins) or space for writing in the name and phone number of the physician or other health provider who distributes the material. Internet materials/methods need not meet this requirement.
4. Each entry must include a report on evaluation of the practice improvement that has been accomplished. The report should document the extent to which the patient education activity or material contributed to the improvement in measured patient health behaviors or health. Information on patient satisfaction with the educational process may also be included, however clear documentation of changes in health outcomes (practice improvement) must be included.
5. Individuals or organizations who have been awarded the H. Winter Griffith Award at the Conference on Patient Education or the Conference on Practice Improvement within the last three years are not eligible to submit for this award.
6. Each entry must include a completed application form.

SUBMISSION REQUIREMENTS AND PROCEDURES

3. The winning entrant will be required to provide free information and/or copies of the patient program (eg syllabus) and/or education materials for distribution to attendees at the Conference and for use by the Society of Teachers of Family Medicine and the American Academy of Family Physicians.

REVIEW CRITERIA
Each entry will be judged on the following:
• The extent to which the patient education produced changes in patient knowledge, attitudes, skills or behaviors that contributed to a documented practice improvement such as a changes in patient health or measures of health.
• Effectiveness as documented in evaluation report
• Usefulness of the improvement project and/or patient education activities.
• Overall evaluation

DEADLINE
Your entry must be postmarked no later than August 1, 2008.

NOTIFICATION OF WINNER
The winner will be notified in mid-September 2008.
